Position: Family Navigator

Landmark Staffing is hiring for a critical role in a local homeless shelter. This position requires reporting
to directors regarding case management and resources for residents. There are 2 openings for this
position; one full time and one part time.

Job Requirements:

  • Post-Secondary Education; BA-BS in a related field and/or minimum of three years' experience
  • with a vulnerable population or similar environment.
  • Excellent verbal and written communication skills
  • Compelled and motivated to provide appropriate assistance to population served

Job Responsibilities:

  • Work directly with residents and the community, promoting housing, support services and
  • residential life skills
  • Evaluate and assist residents with the following: Primary Care access and services, the Housing
  • first program, skills training programs, and more
  • Meet with residents on a regular basis, to set and follow up on goals established

Service Coordination and Delivery:

  • Further implement Trauma Informed best practices into a clinical environment, ensuring available
  • services and programming benefit the vulnerable population served
  • In coordination with the Executive Director, maintain Staff and service focus per organization
  • values (Including, but not limited to):
o Housing; Emergency, Rapid re-housing, Permanent Supportive Housing
o Resource referral and usage

Client/Community Resource Connections:

  • Ensure compliance with Agencies regarding Release of Information, HIPPA and PHI
  • Serve as a liaison between clients and the community to assist them in accessing the resources
  • they need
  • Facilitate the process of applying for food stamps, Medicaid/Medicare, Disability, LEAP, etc.
  • Coordinate and interact with outside agencies meeting with residents

Life Skills Coaching:

  • Provide coaching to clients on an as needed basis.
  • Communicate with PWFC team and Executive Director to make them aware of client plans and
  • current goals
  • Document client demographic information and maintain accurate, current client files
